Pregnancy after laparoscopic myomectomy safe and feasible

NEW YORK (Reuters Health) - Laparoscopic myomectomy can restore fertility and improve the chances of successful pregnancies and term deliveries, Italian researchers report in the July issue of Fertility and Sterility. This approach is relatively safe in the hands of expert surgeons, they say.

In a study of 158 pregnancies achieved after a total of 514 myomectomies performed at the University of Bologna, Dr. Renato Seracchioli and colleagues report a high success rate with no cases of uterine rupture.The average patient age at surgery was 34 years old, with more than half of the women having at least three myomas of 5 centimeters or larger. The average time of conception was 18 months after surgery.

There were 106 deliveries in the series. The spontaneous abortion rate was 27.2%, with most occurring during the first trimester, which is similar to other case reports after laparoscopic myomectomy, but higher than the spontaneous abortion rate of 10% to15% in the general population. Dr. Seracchioli's group found no correlation between myoma location and spontaneous abortion.

 

There was a high rate of cesarean section (74.5%), but the majority of procedures (58.5%) were elective, not because of intrapartum complications. The researchers speculate that is was probably due to a "defensive attitude toward the birth process which is perhaps not necessary." However, the investigators point out that they are surgeons, not obstetricians, and did not closely follow the pregnancies.

Although there were no cases of uterine rupture, Dr. Seracchioli's group emphasizes that "a correct reconstitution of the incision is fundamental."

These preliminary results with laparoscopic myomectomy "gives the possibility not only to restore reproductive capacity, but also to enable the patient to take a pregnancy to a successful conclusion."
