Frequent physiotherapy for patients suffering back pain works no better than a one-off advice session, research showed today.
Telling patients with mild to moderate low back pain to remain active appeared to be just as effective as providing regular appointments with a physiotherapist, according to the study.
Physiotherapists working in the NHS treat around 1.3 million people for low back pain in Britain each year, but there is little evidence for the effectiveness of routine physiotherapy.
The latest research, published in the British Medical Journal, suggests that in many cases a single advice-giving session with a physiotherapist may be all that is needed to help most patients.
The researchers, from the University of Warwick, studied 286 patients who had suffered low back pain for more than six weeks.
Of these, 144 were given therapy and 142 received advice alone.
Patients in the therapy group underwent a physical examination by a physiotherapist who then chose a treatment strategy based on their findings, which included joint mobilisation and manipulation, stretching and heat or cold treatment.
The advice group had a single session, lasting up to an hour, with the physiotherapist who carried out a physical examination and gave general advice to remain active.
The researchers measured the levels of the participants' disability at two, six and 12 months and also asked the patients whether they felt they had benefited.
While the patients in the therapy group were more likely to report benefits, there was no evidence for a long-term effect of the physiotherapy.
After 12 months there was no difference in the disability scores between the two groups.
The researchers concluded: "Routine physiotherapy for mild to moderate low back pain generally practised in the UK is no more effective than a session with a physiotherapist that includes advice to remain active."
They said research suggested that exercise programmes to improve cardiovascular or muscular strength were more beneficial than the routine physiotherapy commonly used in the UK.
