Preterm Birth Risk Higher if Mom’s Asthma Untreated
Women with untreated asthma during pregnancy are at risk of preterm birth, researchers reported.
Dr. Michael B. Bracken of Yale University in New Haven, Connecticut, reported results of a study of 735 women with asthma and 1,848 women without asthma who were followed throughout their pregnancies.
Preterm labor and delivery--defined as prior to 37 weeks gestation--occurred in 10.6% of pregnant women with asthma and in 7.4% of women without the respiratory ailment, Bracken reported.
"The theory is that women who have hyperresponsivity of smooth muscle in the (lungs) also have it in the uterus," Bracken explained. In terms of the link between asthma and preterm delivery, "we don’t think that one causes the other, but that there is an underlying susceptibility for both."
Women with severe asthma actually have a lower risk of preterm birth because they are more likely to have their disease controlled with drugs that also reduce risk of preterm labor, according to Bracken. "The risk is only increased in untreated women with asthma," he said. "The same drugs used to control preterm labor, the beta-2 agonists, are the same that are used to treat asthma, only they are in larger doses for preterm labor," the Yale researcher noted. "The smaller doses used for the woman with asthma gives her substantial protection."
The findings "strongly support" the guidelines for the treatment of asthma in pregnancy established by the National Asthma Education Program, Bracken said. The guidelines recommend the use of beta-agonists and other medications to control the symptoms of asthma during pregnancy.
In a second study, in which Bracken was a co-investigator, Yale researchers found that in a study of 761 pregnant women with asthma, that 65% were undertreated for 3 or more months of pregnancy. Beta agonists were used in 23% of women and anti-inflammatory agents were used in 11%. Clinic patients with mild asthma were less likely to be undertreated compared with their counterparts, while pregnant women with severe asthma attending clinics were more likely to be undertreated.
The Yale team told meeting attendees that, overall, they found that pregnant asthmatics are being undertreated according to the National Asthma Education Program guidelines.
