The Fundamentals of Stroke
A stroke occurs when a blood vessel in or around the brain becomes plugged or ruptures. These blood vessels can be thought of as pipes, and the pipes can get clogged or can leak. When this happens, the area of the brain supplied by the affected blood vessel fails to work properly, and in some cases, that part of the brain dies. When parts of the brain fail to work properly, it produces symptoms of a stroke. These symptoms include weakness, numbness, or difficulties in speaking and walking.
There are different types of stroke. The most common type of stroke occurs when a blood vessel in or around the brain becomes plugged. This is called a cerebral infarction or cerebral ischemia and accounts for about 80% of all strokes. The plug can originate in an artery of the brain or it can originate somewhere else in the body, often the heart, where it breaks off and travels up the arterial tree to the brain, until it lodges in a blood vessel. These "travelling clots" are called emboli. Strokes caused by emboli from the heart are often seen in people with an irregular heartbeat (a condition called atrial fibrillation), as well as after a heart attack or heart surgery. Ischemic strokes are largely preventable if risk factors are recognized early and managed. [Please see our article, "Reducing Your Risk of Stroke."]
Rupture of a blood vessel can produce a bleeding type of a stroke. We call this a cerebral hemorrhage. This is when an aneurysm, or an out-pouching, of a blood vessel in the brain ruptures. These types of strokes only account for about 15-20% of all strokes, but they are devastating. The main thing you can do to prevent a cerebral hemorrhage is to control high blood pressure.
On average, someone has a stroke in the United States every minute. Furthermore, someone in this country dies of a stroke every three-and-a-half minutes. For various reasons, stroke kills more women than men. Each year, stroke kills about 56,000 men but almost 87,000 women. This is partially due to the types of stroke women have as well as the advanced age of women who experience a stroke.
There is a geographic area in the United States, referred to as the stroke belt, in which there is a particularly high incidence of stroke. The stroke belt is in the Southeastern United States. It has the highest national incidence of death due to stroke. Much of this increased incidence of stroke in the stroke belt is due to the presence of increased risk factors, such as high blood pressure, or hypertension. Indeed, there is more hypertension, and more severe hypertension, in the stroke belt than anywhere else in the country. Other factors contributing to the increased incidence of stroke in this area are dietary habits, relative populations of racial groups, and people’s access to health care.
Symptoms of Stroke
The symptoms of a stroke that a person experiences reflect what area of his/her brain is lacking blood flow. Symptoms include weakness or numbness involving one side of the body; trouble talking or walking; sudden problems with vision or eyesight, such as going blind in one eye or blurry vision; and severe headache. These symptoms may start suddenly, but can begin gradually. In many cases, these symptoms come and go. They last for a few minutes and then go away. We call these mini-strokes or TIAs - transient ischemic attacks. It doesn’t matter whether these symptoms start suddenly or gradually, whether they come and go, or just come and stay. The bottom line is that when these symptoms occur, you must call 911. This is the fastest way to get into the medical system.
Importance of Early Recognition
It is extremely important to realize that most strokes do not hurt. Without this pain factor, many people with stroke symptoms--weakness, numbness, speech problems, for example --are not motivated to go the emergency room. A large international study of over 20,000 stroke patients found that only four percent went to the emergency room within three hours of onset of symptoms. Why is it so important to recognize the symptoms of stroke and to get yourself or your loved one into the medical system as rapidly as possible? There are new medications available and in development that can reverse the effects of stroke. However, some of these drugs need to be given within a short time window from the onset of stroke. Otherwise, they may not work.
It is often said that when it comes to stroke, time is brain. This means that any delay in getting a stroke victim to a hospital may result in greater damage to the brain tissue. This of course, may potentially lead to greater disability. Therefore, we need to make sure that we recognize the symptoms of stroke as early as possible.
