Is my daughter's speech developing properly?
All children learn to understand language before they learn to speak it themselves, in a fairly standard developmental sequence.
But there can be huge differences between children who are all developing normally but learn language at different paces depending on what else they are learning at the same time and where their interests and skills lie.
In working out whether your daughter's speech could be delayed you firstly need to look at what she can understand.
As a rough guide you would expect a child of 20 to 24 months to be able to understand sentences and to recognise objects and pictures; in total knowing at least 50 to 100 words. In terms of what she can say her vocabulary should be several hundred words, mostly in sentences.
She should able to ask for anything she needs and tell you about things that have happened. She should start a lot of the talking and be constantly telling you things instead of waiting for you to ask her what she wants or to name objects for her.
Check what her level of understanding language is because if she understands everything you say to her, her expressive speech may be just about to break forth.
Whenever a child appears to speak less than you would expect it is important to make sure she is hearing normally. You are right to ask about her ears as any problem with these could also affect her balance and lead to her being accident prone.
Her dribbling can be normal at this age or could be a sign of enlarged tonsils or difficulty coordinating the mouth muscles to swallow which would also affect her ability to make clear sounds needed for speech.
Talk to your health visitor or GP who can fully assess your daughter's language development, check her ears and arrange a hearing test. They can also check she is developing appropriately in other respects such as her physical abilities including walking and hand-eye co-ordination and her social and behavioural skills.
If she has poor hearing she will need referral by your GP to an ear, nose and throat surgeon who can check for glue ear (chronic inflammation of the middle ear which can affect both hearing and balance) and treat her as appropriate.
If your daughter is hearing normally and has an appropriate level of understanding language, her expressive speech should come into its own in the next six months. You can help by talking to her as much as possible, encouraging her to name objects and to ask for rather than point to something if he wants it.
Mixing with other children of similar age or slightly older is a great way to stimulate her language development. This can be for example at playgroup, nursery or friends' houses.
If your daughter's language is significantly delayed or she has problems in other areas of development she may be referred to the child development team which includes a speech therapist for further assessment and treatment.
