Tooth grinding (also called bruxism) is not unusual in toddlers, especially at night time. It tends to start when their teeth are erupting and most children stop grinding their teeth after the baby teeth have all come through (by three to six years of age).
The loudness of the grinding does not necessarily relate to the wearing out of the baby teeth and it is usually not a serious problem. Although your daughter's baby teeth may become worn down her adult teeth are usually unaffected.
It can become a persistent habit in some children. Occasionally there may be an underlying health problem such as allergy, recurrent ear or throat infections, recurrent fevers, headaches, tooth decay or digestive problems.
You mention that your daughter suffers from ear and nose infections. These may be contributing to her tooth grinding and she probably should be referred to an ear, nose and throat specialist for a general assessment including checking her hearing and to see if she requires further treatment.
In more severe cases tooth grinding can lead to malocclusion or dental cross-bites, if undetected these can also cause problems with the jaw joint; the temporomandibular or TM joint.
If she has had the habit for several years it would be a good idea to ask her dentist to check your daughter at her next visit to make sure her back teeth are not severely worn and to make sure she has a normal bite.
Tooth grinding is more common in children with developmental problems such as cerebral palsy. However there would be additional more obvious signs such as delay in normal development which would have come to light in your daughter by now.
If there is no underlying cause usually no treatment is necessary and your daughter should grow out of it by the age of six or so.
Sometimes your dentist may recommend a simple mouth guard to be worn during sleep, although such guards are often less well tolerated by children of your daughter's age.
