Most children develop a fairly clear sense of whether they are boys or girls somewhere between 18 and 30 months of age.
This sense of being a 'boy' or a 'girl' is called gender identity. Afterwards children develop gender stability. This is when they realise that little girls grow up to be women with breasts and vaginas and that little boys grow penises and turn into men.
During this period of gender stability many children develop stereotypic behaviours for example by starting to dress and play in ways appropriate to their gender identity.
For most children their gender identity matches with their biological gender (i.e. their sex as determined by their chromosomes and hormones). There is however a rare condition called gender identity disorder (GID) in which the biologic gender is not the same as the gender identity of the person.
Understandably it can cause a lot of heartache and confusion for the affected child and his parents. I use the word 'his' because research shows that most children referred to clinics for evaluation of possible GID are boys - almost six to one boys to girls in one study.
Of course part of the reason for this difference could be that our society is much more accepting of girls being 'tomboys', dressing like boys and engaging in 'boyish' activities, than it is of boys behaving like girls. As a result parents may pick up differences in their son's sexual behaviour much sooner than they would for a daughter and seek help earlier.
Children with GID may show several of the following:
• repeatedly stated desire to be, or insistence that he/she is, the other sex
• in boys, preference for cross dressing or simulating female attire, in girls, insistence on wearing only stereotypical masculine clothing
• strong and persistent preferences for cross-sex roles in make-believe play or persistent fantasies of being the other sex
• intense desire to participate in the stereo-typical games and pastimes of the other sex
• strong preference for playmates of the other sex
They may have a persistent discomfort with their own sex or a sense they are in the wrong gender role for that sex. For example boys might find their penis and testes disgusting or be convinced they will disappear as they get older or wish they did not have a penis.
Your son is only five and may still be developing his gender stability. However the fact that he keeps insisting he is a girl and wants to wear her clothes suggests he may have GID.
If he is not distressed by how he feels and is not experiencing any problems at school or with friends then it may be that all you need to do at this stage is to put his behaviour to one side and focus on your relationship with your son.
Praise him for all his achievements, however small, spend time playing with him in his preferred activity and make him aware how much he matters to you and that you love him for himself.
The times to seek help are if:
• your son seems to have difficulty relating to other children, if he is being bullied or if he is not doing as well as you would expect at school
• your son's mood or behaviour changes, for example if he seems unhappy, becomes withdrawn, or if he becomes persistently naughty or difficult for no obvious reason
• if your own family is undergoing emotional problems or stress for example if there has been a recent bereavement of someone your son was close to, or a family break-up or severe illness in the family; all these factors can affect his gender development.
• If you are worried, even though your son seems unworried by his behaviour, further expert advice may help you support your son in the right way as he grows up.
It would be helpful at this stage to discuss your concerns with your GP who may examine your son to make sure he does not have any obvious physical intersex conditions (these are all very rare). Your GP or health visitor can refer your son to a specialist gender identity clinic.
The clinic which deals with gender identity problems in the UK is the Gender Identity Development Service based in London at the Portman and Tavistock NHS trust (details below). This specialist service is a National Health Service open to people registered with a doctor anywhere in the UK.
It is staffed by a multi-disciplinary team made up of child and adolescent psychiatrists, psychologists, social workers, psychotherapists and paediatricians. Everyone works closely together to work out the best way to help each child or adolescent - the Service accepts self referrals of families or adolescents beyond the age of sixteen.
