Why is my daughter passing strangely coloured stools?
It is quite normal for a child to pass different coloured stools from day to day. One day they may be much darker than another day. However you need to seek medical advice as your daughter's stools have been persistently pale or white for more than a couple of days.
The unusual colour of your daughter's stools suggests that her gall bladder is not emptying bile (the dark green pigment which result from the breakdown of blood pigments) into her bowels. Without this pigment her stools will look pale.
If the bile pigments are not excreted into the bowel they will build up in your daughter's blood stream. The result of this is that she may look jaundiced, feel itchy and pass dark coloured urine as she will excrete the excess bile into her urine.
You need to see if your daughter is jaundiced. Check to see if her skin or the whites of her eyes look yellow. Check your daughter's urine to see if it looks darker than usual - often it looks the colour of tea. This condition is called 'cholestasis'. There can be many reasons.
One possible reason is that your daughter has caught hepatitis A. This is a viral infection which causes her liver to become inflamed. Young children such as your daughter can be surprisingly well. She may have suffered with mild flu like symptoms and diarrhoea for a week or two before her stool became pale.
Just as she seemed to be picking up and her appetite started improving you would have noticed her stools turning pale. This is also the stage when she might look yellow and pass dark urine.
Older children and adults often feel very itchy, complain of tummy ache and redevelop a fever and flu like symptoms, but children of your daughter's age are less likely to feel ill at this stage. Most children of her age make a full recovery within a month.
Other viruses can cause a similar hepatitis. Cholestasis can also be due to congenital problems with the liver, gall bladder or bile duct. However in such cases your daughter would have probably had long standing health problems presenting in her first year of life whereas it sounds as if she has been well up until now.
Your GP needs to see her as soon as possible for a full examination. Take a urine and if possible stool sample to be checked. Your daughter will probably need a blood test to see if her liver is inflamed and to check whether she has caught hepatitis A or another infection.
If she does have Hepatitis A she will have been contagious for a few weeks before her stools turned pale and will continue to excrete the virus in her stools for another week or two. Your GP will inform public health staff who will advise any contacts who may be at risk.
She will need to stay away from playgroup or nursery until she is well. You and the rest of her immediate family should be tested to make sure none of you has caught the virus and will probably then be offered vaccination with Hepatitis A vaccine if you have not already had it.
If your daughter becomes unwell or her problems are not due to Hepatitis A, your GP will probably refer her to the local paediatrician for a full assessment and any necessary treatment.
