Aneurysms: years without symptoms, then they erupt

Is it just a headache, a migraine or an aneurysm?

They can be hard to differentiate. But a headache signaling a ruptured aneurysm feels out-of-the-ordinary, tremendous and does not go away, and it tends to be spread over most of the head.

Can you cut your risk?

Two big risk factors you can control: :: Don't smoke. :: Guard against high-blood pressure.

Family history of aneurysms is also a risk factor.

It's a volcano waiting to erupt in the brain.

When an aneurysm ruptures, it can result in brain damage, coma or death.

It landed actress Sharon Stone in a hospital last month. A 20-year-old woman died of a ruptured brain aneurysm after riding Montezooma's Revenge roller coaster two months ago at Knott's Berry Farm in California.

In March 2000, Eric Samaniego, a critical-care nurse from Yorba Linda, Calif., had a tremendous headache — something he had never experienced. "My vision started getting blurry, and I started to lose my balance." As a nurse, he knew something was terribly wrong.

 

His wife called 911. Samaniego remembers little about the month after he arrived in the emergency room. He was told the aneurysm was corrected through a series of operations. But the rehabilitation and recovery were tough.

Samaniego had blurry, double vision and could not walk or write for nine months. His speech was affected. And the worst part, he said, was that he could not interact as he used to with his children and wife.

Today, Samaniego is back at work, nearly fully recovered except for lingering minor speech difficulties. He considers himself fortunate to have known that the severe headache last year was a sign of brain injury.

But aneurysms can go for years undetected and without symptoms.

Q: What is a brain aneurysm?

A: It's a bulge in the blood vessels of the brain. It is caused by a weakness in the wall of the vein or artery. People can be born with it and not know it exists. Others develop it over the years.

Q: Do aneurysms grow?

A: Yes, they do. Aneurysms bigger than 1 centimeter in diameter are considered giant and can put pressure on the brain. But an aneurysm does not have to be giant to rupture.

Q: What makes an aneurysm rupture?

A: Any event that makes blood pressure rise. It can happen in a moment of anger, rage, anxiety or joy, while on a roller coaster, while having sex, during a sneeze or cough.

Q: What are the symptoms?

A: A rupture occurs suddenly. Sometimes, there are no symptoms. In other cases, a person may experience a sudden, tremendous headache that won't stop, nausea, impaired vision, dizziness, vomiting and loss of consciousness.

Q: What are the effects of a ruptured aneurysm?

A: Blood can bleed into the brain or in the areas around the brain, resulting in a clotted mass. Brain and spinal fluid can accumulate. Rebleeding, blood-vessel spasms and more aneurysms can occur. The result can be brain damage, coma or death.

Q: What should I do if I believe a loved one or I have symptoms of a ruptured aneurysm?

A: See a doctor immediately.

Q: How are ruptured aneurysms treated?

A: There are several ways, but two surgical techniques often are used: One is clipping the ruptured aneurysm. Another is to fill the aneurysm with platinum coils to prevent it from filling with blood.

Q: What is done to aneurysms that haven't ruptured?

A: It depends on the condition of the aneurysm and the health of the patient. Some aneurysms may be monitored, others treated with the previously mentioned surgeries.
