Novel Oral Platelet Growth Factor, Eltrombopag, To Be Evaluated In Long-term Idiopathic Thrombocytopenic Purpura (ITP) Study

GlaxoSmithKline plc has announced the initiation of the first global randomized, double-blind and placebo-controlled Phase III trial that will assess the safety of using eltrombopag for up to six months in chronic idiopathic thrombocytopenic purpura patients. The trial, called RAISE (RAndomised placebo controlled ITP Study with Eltrombopag), will involve 189 patients who will be enrolled at 135 centers in 26 countries. Eltrombopag is an investigational agent that is not approved for use in any market for any indication at this time.

The initiation of this trial follows on from promising results seen in earlier trials. In one six week study, at eltrombopag doses of 50mg and 75mg, 70% and 81% of patients respectively achieved the primary clinical endpoint of having a platelet count greater than 50,000/ mL compared to 11% of patients on placebo.(1) In that study 47% and 61% of patients experienced at least one adverse event (AE) during treatment with eltrombopag at doses of 50mg and 75mg compared to 59% of patients receiving placebo.(1) The most common AE observed during the study was mild to moderate headache, reported in 21% subjects on placebo, 10% on eltrombopag 50mg, and 21% on eltrombopag 75mg.(1) There was no apparent relationship between dose and incidence of AE's observed in this study.

"We are extremely encouraged by the results from earlier eltrombopag trials", said Paolo Paoletti, MD, Senior Vice President, Oncology Medicine Development Centre, GSK. "As we continue development in Phase III trials we are very optimistic about eltrombopag in a variety of conditions where thrombocytopenia is a complicating factor. The results of this trial will represent a significant milestone for GSK and underscore our continued commitment to delivering novel treatments to potentially meet a significant unmet medical need."

Idiopathic thrombocytopenic purpura is an autoimmune disorder that is marked by increased platelet destruction and/or inadequate platelet production. This imbalance between platelet production and destruction can result in unusually low platelet counts(1) leaving patients at risk of spontaneous bruising, mucosal bleeding, and in severe cases, gastrointestinal (GI) bleeds and intracranial hemorrhage. Normal platelet counts are in the range of ≥150,000/uL and ≤400,000/uL.(2)

Current ITP treatments work to decrease platelet destruction by suppressing the patient's immune system, which can lead to increased risk of infections.(3) Eltrombopag appears to work through an entirely different mechanism - increasing platelet counts by boosting platelet production.

Eltrombopag is an investigational non-peptide small molecule that is administered orally. It is believed to function by interacting with the thrombopoietin (TPO) receptor, which is located on the surface of specific cells in the bone marrow called megakaryocytes. Megakaryocyte stimulation leads to the production of platelets, thus increasing platelet counts.

"It is exciting news that a new drug is in development which could offer chronic ITP sufferers a longer term treatment option," says Joan Young, President of the Platelet Disorder Support Association. "Current treatment options such as steroids have drawbacks if used on a long-term basis, and the more radical option of removing the spleen comes with a lifelong increased risk of severe infection. Eltrombopag may offer real hope to patients who are in need of new therapies."

Study Design

The RAISE trial will be conducted at approximately 135 investigative sites globally and 189 adults with chronic ITP will be enrolled. Eligible patients must have received one or more prior ITP therapies. The study is designed to assess the safety, efficacy and tolerability of eltrombopag for up to six months in previously treated patients with chronic ITP.

The primary outcome in the study is subjects with platelet counts between ≥50,000/uL and ≤400,000/uL during the six month treatment period. Secondary outcomes also duration of platelet count between 50,000/uL and ≤ 400,000/uL, incidence and severity of bleeding, as well as the frequency of adverse events.

About eltrombopag

Eltrombopag is an investigational thrombopoietin receptor agonist that has been shown in pre-clinical research and early phase clinical trials to stimulate the proliferation and differentiation of specific bone marrow cells that give rise to blood platelets. Therefore, eltrombopag is considered a platelet growth factor. Eltrombopag is administered orally as a tablet and may have less potential than large protein molecules for causing an immune system reaction. Eltrombopag was discovered via a research collaboration between GlaxoSmithKline and Ligand Pharmaceuticals. It is being developed solely by GlaxoSmithKline.

About thrombocytopenia and ITP

A normal blood platelet count is ≥150,000/uL and ≤400,000/uL.(2) A reduction in platelet count (to a level <150,000/mL) is the defining characteristic of thrombocytopenia and the diagnosis can be confirmed following a routine blood test. Thrombocytopenia occurs in 5% to 10% of all patients hospitalized for any cause. (4)

ITP is an autoimmune disorder that is marked by platelet destruction and/or inadequate platelet production. Primary ITP is estimated to affect 50 to 100 new persons per million per year in the United States and Europe.(5) Some patients with ITP are asymptomatic or have mild bruising while others develop mucosal bleeding that can become severe,(6) including intracranial hemorrhage.(3)

Thrombocytopenia can occur as a consequence of chemotherapy treatment, interferon treatment, viral infection or chronic liver disease. Thrombocytopenia can impede a variety of medical treatments. It can prevent cancer patients from receiving their full dose of chemotherapy, preclude patients with hepatitis C infection from receiving interferon therapy or lead to dose reductions or discontinuation, and complicate surgical or dental procedures.
