Decisiones para los pacientes del cáncer de la piel - procedimiento sistemático de la terapia para identificar marcadores pronósticos del tumor

Para seleccionar la estrategia terapéutica óptima para el melanoma malo, la forma más seria de cáncer de la piel, es crucial poder discriminar, con altas confiabilidad, etapas del tumor y ocasiones de la repetición con la ayuda de los marcadores del tumor y predecir la progresión de la enfermedad sobre esta base. En colaboración con varios grupos de trabajo basados en Nottingham, Inglaterra, profesor Dirk Schadendorf, jefe de la unidad clínica “Dermato-Oncología” de la cooperación del centro de investigación alemán de cáncer, Heidelberg (Deutsches Krebsforschungszentrum, DKFZ) y su compañero de trabajo, el Dr. Selma Ugurel, han investigado un método para identificar marcadores etapa-específicos del tumor. En el diario de la oncología clínica los científicos presentan las ventajas de su método que combine una forma de espectroscopia total con algoritmos bioinformatic.

Hasta ahora, ha habido solamente pocos marcadores del tumor disponibles para predecir la progresión de la enfermedad en melanoma. S100-ß, una molécula que se pueda identificar por mancharse inmune, es de uso frecuente como indicador del cáncer avanzado, metastasizing. Los investigadores de Heidelberg, en común con colegas del Reino Unido, ahora han probado un nuevo acercamiento: Generaron perfiles de todas las proteínas en las muestras del suero de sangre de 205 pacientes del melanoma en etapas I e IV. Estos resultados fueron combinados con un software analítico llamado red neuronal artificial (ANA). Después de un “proceso de aprendizaje”, ANA podía distinguir sistemáticamente a pacientes de la etapa I de los pacientes de la etapa IV basados en los marcadores del tumor. En analogía a esto, llegó a ser posible predecir la ocasión de la repetición en pacientes de la etapa III después del retiro quirúrgico de los tumores primarios y secundarios. Las predicciones basadas en la combinación de MALDI-ToF y de ANA resultaron ser confiables en 82 por ciento de casos, mientras que S100-ß facilitó la diferenciación correcta en solamente 21 por ciento de casos. Si el procedimiento prueba su valor, los perfiles de la proteína pueden en futuro contribuir a identificar a pacientes de riesgo elevado y a referirlos a la terapia ayudante más rápidamente.

En Alemania, diagnostican a aproximadamente 11.400 personas - cerca de 6.100 mujeres y 5.300 hombres - con el melanoma malo cada año. El melanoma malo explica así casi tres por ciento de todos los neoplasias y causas malos cerca de un por ciento de todas las muertes del cáncer (en las mujeres de 2000:1,017 y 1.161 hombres).

La tarea del Deutsches Krebsforschungszentrum en Heidelberg (centro de investigación alemán de cáncer, DKFZ) es investigar sistemáticamente los mecanismos del desarrollo del cáncer e identificar factores de riesgo del cáncer. Se espera que los resultados de esta investigación básica conduzcan a los nuevos acercamientos en la prevención, la diagnosis y el tratamiento del cáncer. El centro es financiado a 90 por ciento por el ministerio federal de la educación y de la investigación y a 10 por ciento por el estado de Baden-wurttemberg. Es un miembro de la asociación de Helmholtz de los centros de investigación nacionales (Helmholtz-Gemeinschaft Deutscher Forschungszentren e.V., HGF).
