FDA Advisory Panel Recommends Approval Of J&J Lymph Node Test For Breast Cancer Patients

An FDA advisory panel on Thursday recommended that the agency approve with certain conditions Johnson & Johnson subsidiary Veridex's test called GeneSearch Breast Lymph Node Assay, which aims to detect cancerous growths in lymph node tissue removed from breast cancer patients, Reuters reports (Reuters, 11/16). The test also aims to help physicians decide whether more lymph nodes need to be removed. The test uses Cepheid's SmartCycler system to identify cancerous growths that are at least 0.2 millimeters in size by detecting the genes Mammaglobin and Cytokeratin 19, which usually are seen at low levels in normal lymph node tissue, according to Veridex (Reuters, 11/15). The panel, which voted 9-1 to recommend GeneSearch, said the test could provide doctors with more information to treat women with breast cancer. The panel also questioned the test's ability to determine the most appropriate treatment for patients, according to Reuters. "I think what you're hearing from the panel is, 'Yes, let's proceed forward but cautiously,'" panelist Marc Ernstoff, a cancer immunologist at Dartmouth-Hitchcock Medical Center, said (Reuters, 11/16). Prior to the advisory panel decision, FDA on Wednesday questioned the company's lack of long-term survival data and also the company's exclusion of invalid test results when analyzing data on the efficacy of the test, according to Reuters.

"It is unclear to us what the implications of exclusion or inclusion have on the clinical utility of assay results," FDA staff wrote. Veridex in a statement said, "The benefits to a breast cancer patient of rapid, objective detection of clinically relevant ... metastases in the lymph node tissue outweigh any potential adverse event or risk to the patient or user due to assay malfunction or operator error." According to Veridex spokesperson Steve Dnistrian, GeneSearch recently received approval in Europe. Dnistrian declined to comment on the FDA documents and the potential market for the test, Reuters reports (Reuters, 11/15).
