Use Of Avagard In Pediatric Urologic Procedures

UroToday.com - A study performed by Jeffrey Palmer evaluated the use of Avagard compared with hand-brush scrub preparation for preoperative scrubbing in regards to inpatient and outpatient pediatric urologic operations. Avagard is a waterless, brushless, and scrubless hand antiseptic indicated as a replacement for traditional preoperative brush hand scrubbing. It is composed of chlorhexidine gluconate 1% solution along with ethyl alcohol 61% wt/wt.

The study evaluated the first 550 patients where Avagard was utilized as a preoperative hand antiseptic and compared these 550 patients with a previous 550 consecutive patients had traditional antiseptic-impregnated hand-brush scrubbing. All patients underwent pediatric urologic procedures that were similar in nature. Patients were monitored postoperatively for signs of wound infection, and patients and the surgeon were monitored for side effects. A cost analysis was performed to compare the use of Avagard as a preoperative hand preparation and comparing it to the antiseptic impregnated hand-brush preparations.

The group found that the incidence of wound infection was not statistically significant between the two groups. There was one wound infection in the Avagard group and two in the hand-scrub group. All wound infections were successfully treated with a single course of oral antibiotics without any long-term sequelae. No side effects for the patients or the surgeon were noted. They did find that the use of the Avagard was more time efficient and cost effective.

The group concluded that Avagard is a fast and effective way to apply an antiseptic prior to surgery. It seems to be much more cost effective as well as time efficient when compared to traditional surgical scrub. In my opinion, if it is just as good and faster then why not use it. It may become extremely helpful for cases requiring a very fast surgical preoperative scrub such as trauma cases.

Recurrent Sexually Transmitted Infections In Men After Their Initial Episode: New Study Suggests A Need For Repeat Screening After Initial Infection

UroToday.com - In women, repeat screening after initial STD treatment is customary due to an incidence of asymptomatic re-infection approaching 10-15 percent. In men, however, currently no recommendations exist regarding the need for repeat testing after initial single dose treatment of STDs.

In the October 17th issue of the Annals of Internal Medicine, Peterman and colleagues from the RESPECT-2 Study Group report an analysis of data collected from a randomized controlled trial performed in 3 urban STD clinics designed to assess the incidence of new STDs within a year of the initial infection.

2419 patients visiting an STD prevention clinic were followed for one year for a total of 8129 3-month follow-up visits. Of 1236 women studied, nearly 26% exhibited 1 or more infection, consisting most commonly of Chlamydia (11.9%), Gonorrhea (6.3%), and Trichomonas (12.8%). Infections were seen in 14.7% of men studied (n=1183), most commonly consisting of Chlamydia (9.4%) and Gonorrhea (7.1%). Those patients infected at baseline had the higher risk of infection at subsequent visits within 6 months (16.3 per 100 3-month intervals) and at 9 and 12 months (12 per 100 3-month visits). Remarkably, even in men, 66% of infections were asymptomatic.

While traditionally recurrent STDs are thought to be asymptomatic mostly in women, these data suggest that repeat, asymptomatic infection may be common among men, warranting repeat screening 3 months after treatment even in asymptomatic patients.
