List of worst U.S. nursing homes published

NEW YORK (Reuters Health) - Before placing a loved one in a nursing home, you might want to consult the U.S. Centers for Medicare and Medicaid Services' (CMS) newly released list of the nation's chronically poor-performing nursing homes. The report cites 54 of them in 33 states and the District of Columbia.

The list of these "special focus" facilities -- a designation used by CMS to identify nursing homes that need additional oversight because they continually fail to meet safety and quality of care standards in their states -- is available at www.cms.hhs.gov/CertificationandCompliance/12_NHs.asp#TopOfPage.

According to a statement issued by the CMS Media Affairs Office, as of October 2007, there were 128 special focus facilities, out of about 16,000 active nursing homes operating in the U.S.

The number of special focus facilities in each state varies according to the number of nursing homes in the state. These facilities, at the time of their selection as a special focus facility, had survey results that were among the worst 5 percent to 10 percent in each state, according to CMS.

The updated list includes 54 facilities that are at the top of the poorest performers in those states. These facilities consistently provided poor quality care, yet periodically made just enough improvement to pass inspection, and then reverted back to failure with many of the previously noted problems, CMS notes.

These facilities with a "yo-yo" compliance history often fail to address underlying problems, giving rise to repeated quality of care infractions, according to the agency.

CMS Acting Administrator Kerry Weems hopes publication of the poor-performers will "promote more rapid and substantial improvement in the quality of care in identified nursing homes and end the pattern of repeated cycles of non-compliance."

For beneficiaries and their families looking for a nursing home, CMS suggests visiting the facility and talking to the staff, residents and other families. The agency also suggests asking for the results from the latest state or CMS quality survey and visiting www.medicare.gov/NHCompare.

It's also a good idea to call the state survey agency to learn more about the nursing home. If the facility is in the special focus program, find out how long it has participated. Facilities in the program for 18 to 24 months are either close to "graduating" due to significant improvements to care or ending their participation in Medicare and Medicaid.
