Verapamil can slow Peyronie's disease progression

Last Updated: 2007-07-26 10:54:42 -0400 (Reuters Health)

NEW YORK (Reuters Health) - Injections of verapamil directly into the penis in patients with Peyronie's disease resulted in a reduction in curvature of the penis in 18 percent of the patients and disease stabilization in 60 percent, according to the results of a new study.

Described in the18th century by French physician Francois Gigot de la Peyronie, Peyronie's disease is an inflammatory condition caused by the build-up of plaque or scar tissue beneath the skin of the penis. This results in curvature, pain and impaired sexual performance, according to the American Urological Association.

Symptoms of Peyronie's disease vary widely, but painful erections and difficult intercourse are usually the reasons patients seek medical help, the AUA says. The condition is estimated to affect 4 out of 100 men, but because many men are reluctant to discuss it, the number may be higher.

Peyronie's disease is usually treated with oral medications and local drug injections for the first few months. Surgery may be considered for men who do not respond to these treatments and for the most severe cases.

In a study published in the medical journal Urology, Dr. John P. Mulhall and colleagues at Weill Medical College of Cornell University, New York, investigated the impact of injecting verapamil the into the penile plaque in 94 men with Peyronie's disease.

Verapamil is a heart drug used to treat irregular heartbeats, high blood pressure and chest pain (angina). It relaxes the blood vessels, increasing the supply of blood and oxygen.

The average patient age was 44 years old and the average duration of disease was 5.7 months. All of the men had penile curvature. They received six injections of verapamil given two weeks apart.

At follow-up 5.2 months after the last injection, which was approximately one year after disease onset, the researchers found that 18 percent of men had improvements in penile curvature, 60 percent had no disease progression and 22 percent had a worsening of disease.

These numbers are "significantly improved over the number who had stable deformity in our natural history database," the Cornell team writes. The study also demonstrated the safety of the procedure.

Mulhall told Reuters Health that if patients consult the doctor during the early stage of this condition, they have a 50 percent chance that the penile deformity will not become worse.

"Nobody knows exactly how this treatment actually works," he added. "It is possible that the needle or saline that the verapamil is in are just as important as the drug."
