Cocaine craving may be reduced by N-acetylcysteine

NEW YORK (Reuters Health) - New evidence suggests that treatment with N-acetylcysteine can suppress the reaction to cues that stimulate drug-craving in individuals who are addicted to cocaine, according to findings published in the American Journal of Psychiatry.

"We may have a completely new pharmacological mechanism for treating cocaine addiction and perhaps other addictive disorders," Dr. Peter W. Kalivas from the Medical University of South Carolina, Charleston told Reuters Health.

N-acetylcysteine has been used as a mucus-dissolving therapy in patients with respiratory conditions, as a treatment for acetaminophen overdose, and to protect the kidneys from damage by contrast agents used during X-ray imaging studies.

Other uses of N-acetylcysteine that have been investigated include prevention and treatment of cancer, and as an anti-HIV drug.

Kalivas and colleagues assessed the effects of N-acetylcysteine on cue-induced cocaine-craving in a study of 15 addicted individuals.

The researchers found that N-acetylcysteine treatment decreased the patients' rating scores for craving, desire to use, and interest evoked by depictions of cocaine use on photographic slides.

N-acetylcysteine has the potential to "dampen cravings, promote cognitive therapy, and thereby inhibit the vulnerability to relapse in addicts," Kalivas said. "This will involve daily use."

N-acetylcysteine is active in stabilizing the addiction-associated abnormalities of glutamate transmission, he observed. "Even if N-acetylcysteine proves not as useful as we hope, it is the first in what may be a line of compounds with related pharmacological actions."

Another study is being conducted to evaluate the effect of N-acetylcysteine on relapse rates, Kalivas added, and pilot clinical trials are examining the effects of N-acetylcysteine on craving in nicotine and marijuana addiction.
