Warm-up helps asthmatic athletes breathe easier

NEW YORK (Reuters Health) - People with asthma can reduce the airway tightening that can accompany physical activity by warming up with short bouts of high-intensity exercise before a workout, a new study shows.

Exercise-induced bronchoconstriction strikes up to 90 percent of people with asthma, Dr. Timothy D. Mickleborough of Indiana University in Bloomington and colleagues note. Previous studies have found that exercise-induced bronchoconstriction can lessen after repeated exercise in about half of people with asthma.

Athletes may also use drugs called beta-2 antagonists before exercising to reduce bronchoconstriction, the researchers note, but these drugs can become less effective when used every day, and may also worsen lung function with regular use.

To investigate whether repeat, high-intensity warm-ups can help fight exercise-induced bronchoconstriction without drugs, the researchers had eight asthmatic athletes exercise on a treadmill four separate times without warming up (control condition); after performing eight 30-second runs at peak speed with a 45-second break between each interval; after taking 200 micrograms of the beta-2 agonist salbutamol, also known as albuterol; and after sprinting and using the inhaled drug.

With no preparation, exercising produced a roughly 18-percent reduction in lung capacity, the researchers found. But lung capacity fell by just 9 percent with the sprint warm-up, and did not meet the 10-percent drop that defines exercise-induced bronchoconstriction.

The effects of salbutamol plus warm-up were additive. Lung capacity rose by about 9 percent after the athletes took the drug alone, and rose to 15 percent with the drug plus warm-up.

The findings show, Mickleborough and his team conclude, that asthmatic athletes can use high-intensity interval warm-ups to help prevent exercise-induced bronchoconstriction without relying on drugs. "This is particularly important for asthmatic children, since it is important that they exercise without or with minimal medication, since it would make them feel less dependent on it," they write.
