Liver Cancer (Metastatic)

Metastatic liver cancer is a cancer that started somewhere else in the body and has spread to the liver. This type of cancer most commonly starts in the lung, breast, large intestine, pancreas or stomach. Leukemia and lymphoma may also involve the liver. A diagnosis of metastatic liver cancer is sometimes the first way a person finds out he or she has cancer.

Symptoms

Symptoms of the disease include:

  • Weight loss
  • Poor appetite
  • Fever
  • A liver that is enlarged, hard, tender or lumpy
  • Abdominal area may become swollen with fluid (called ascites)
  • Jaundice, which is usually mild at first, worsens in the weeks before the person dies
  • Confusion and drowsiness may result as toxins fill the brain

Diagnosis

It is hard to discover the condition in the early stages. In late stages, tumors cause the liver to stop working normally.

In addition to liver function tests, diagnostic options include:

  • Ultrasound
  • Computed tomography (CT) scans
  • Magnetic resonance imaging (MRI) scans
  • Liver biopsy, which confirms the diagnosis in about 75% of cases

Treatment

Depending on the type of cancer, chemotherapy drugs may be used to try shrinking the tumor and allow the person to live longer, but they do not cure the cancer. Radiation therapy can sometimes reduce severe pain. If there is only a single tumor, it may be removed by surgery, especially if the cancer started in the intestines. In most cases, all a doctor can do is relieve the symptoms.

 

Liver Cancer (Primary)

Primary liver cancers are cancers that began in the liver and did start in another part of the body. The various types include:

  • Cholangiocarcinoma, a slow-growing cancer that starts in the lining inside the liver or in the bile ducts outside the liver
  • Hepatoblastoma, which is more common in infants. Boys get the disease twice as often as girls. It sometimes occurs in older children and may produce hormones that result in early puberty.
  • Angiosarcoma, a rare cancer that starts in the blood vessels of the liver

Symptoms

Signs of primary liver cancers are often vague. They include:

  • Sudden failing health
  • Jaundice
  • Weight loss
  • Discomfort in the abdominal area

Causes and Risk Factors

In most cases of liver cancer, the cause is not known. Long-standing ulcerative colitis and scelerosing cholangitis can sometimes increase the chances of getting cholangiocarcinoma. Angiosarcoma can be caused by exposure to vinyl chloride or arsenic in the workplace.

Diagnosis

Depending on the type, liver cancers can be diagnosed using one or more of the following:

  • A physical exam (doctors can sometimes feel a a large mass in the stomach area)
  • Biopsy (A small sample of liver tissue is taken for examination under the microscope.)
  • Special X-rays

Treatment

If the cancer is detected at an early stage, the surgeon may be able to remove the tumor. Usually, however, treatment of these types of cancers offers little help. Most people die within months of diagnosis.
