Expert anti-diarrhea advice for traveling athletes

NEW YORK (Reuters Health) - Like any other travelers, athletes on the road should be aware of their risk of gastrointestinal infections and take steps to prevent them, a sports medicine specialist advises.

"Traveler's diarrhea (TD) is a common nuisance, but is particularly distressing for athletes because it affects their performance," Dr. Blake Reid Boggess of Duke University Medical Center in Durham, North Carolina writes in the journal, Current Sports Medicine Reports. "Although episodes of TD are nearly always benign and self-limited, the dehydration caused by diarrhea can be detrimental for athletes."

The Centers for Disease Control and Prevention has classified world regions based on TD risk, with visitors to certain parts of Eastern Europe, the Caribbean and South Africa facing intermediate risk, Boggess notes. Most of Asia, the Middle East, Africa and Central and South America are considered high risk areas, he adds.

Along with the well-known precaution of not drinking the water, traveling athletes should also avoid salads, unpasteurized dairy foods, meats and seafood that have not been well cooked, and unpeeled fruits, according to Boggess. Food from street vendors can also be risky.

Fluid replacement is the standard treatment for TD, he writes, with the World Health Organization recommending an oral rehydration solution of salts and sugar along with water. When antibiotics are required, Boggess notes, azithromycin is "an excellent choice"; quinolone antibiotics should be used with care in athletes, he adds, because they can increase the risk of tendon rupture.

While there are effective ways to prevent TD -- taking antibiotics before the trip, for example -- such approaches should be limited to people who face a significant risk if the develop dehydration or other complications of diarrhea. Instead, Boggess says, athletes can try probiotics, which contain "good" bacteria such as Lactobacillus that may prevent bad bugs from invading the intestines. And athletes should skip antimotility drugs to prevent bowel movements, such as Lomotil, unless they have very severe diarrhea and are taking antibiotics.

Athletes planning to travel should be given a prescription for antibiotics, Boggess recommends, and should fill it and start taking the drug if they have diarrhea more than four times daily, if they develop fever, or if they have blood, pus or mucus in their stool.

"Prophylactic antibiotics may be considered for short-term travelers who are high-risk hosts or are taking critical trips during which even a short bout of diarrhea could be devastating to their performance," he concludes.
