Lyrica Significantly Reduced Pain and Helped Patients Manage the Symptoms of Fibromyalgia, Data Show

Lyrica is First Treatment Under FDA Review for Fibromyalgia, Which Affects More than Six Million Americans

BOSTON--(BUSINESS WIRE)--Significantly more patients treated with Pfizers Lyrica reduced their pain by 50 percent or more compared with placebo, according to study results presented today at the American Academy of Neurology annual meeting. Clinically, this outcome would equate to a patient with severe pain reporting a reduction to mild to moderate pain.

Fibromyalgia is one of the most common chronic, widespread pain conditions and is thought to result from neurological changes in how patients perceive pain. Fibromyalgia is usually accompanied by poor sleep, stiffness and fatigue. The pain of fibromyalgia can hamper a patients ability to work and often results in increased medical costs and disability. There are no medications approved to treat fibromyalgia.

A growing body of evidence is defining the biology behind fibromyalgia that causes such devastating and constant pain, said Dr. I. Jon Russell, one of the studys authors and associate professor of medicine in the division of clinical immunology and rheumatology and director of the university clinical research center at the University of Texas Health Science Center, San Antonio. A reduction in pain is critical for people living with this condition. With positive new data and new treatments on the horizon, the outlook for people with fibromyalgia has never been better.

The 14-week placebo-controlled study included 745 patients with fibromyalgia who were randomized to receive Lyrica (300mg, 450mg or 600mg) or placebo daily. Patients were asked to measure their pain on a scale of zero to 10; the baseline score for study participants was 6.7 on this 10-point scale.

The study found that patients receiving 600mg a day of Lyrica reduced their pain by 2.05 on the pain scale; 2.03 for patients taking 450mg a day; 1.75 for patients taking 300mg a day, and 1.04 for patients taking placebo.

Significantly more patients treated with Lyrica reduced their pain by 50 percent or more compared with placebo. Of those patients taking 600mg of Lyrica a day, 30 percent said their pain was cut in half or better; 27 percent of those taking 450mg a day and 24 percent of those taking 300mg also reported this level of pain relief. Of those taking placebo, 15 percent reported pain reduction of 50 percent or greater.

Patients receiving Lyrica also reported significant improvements in overall health status and outcomes, including measures such as physical function and ability to perform everyday tasks.

The most common side effects in the study were dizziness and somnolence, followed by weight gain, headache and peripheral edema.

The results of these data were submitted to the FDA as part of a supplemental New Drug Application for Lyrica for the treatment of fibromyalgia. Pfizer also intends to pursue a fibromyalgia indication in other major markets worldwide.

In the United States, Lyrica (pregabalin) capsules CV is approved for the management of neuropathic pain associated with diabetic peripheral neuropathy and post-herpetic neuralgia, and in epilepsy for the adjunctive treatment of partial onset seizures in adults.

New Treatment for Fibromyalgia

Study Demonstrates Effectiveness of the Antidepressant Duloxetine for Improving Symptoms and Relieving Pain

Fibromyalgia is a chronic, incapacitating musculoskeletal disorder. Nearly six times more common in women than in men, fibromyalgia is marked by widespread body pain and muscle tenderness, often accompanied by headaches, sleep disturbances, and fatigue. While its cause remains a mystery, fibromyalgia has been linked to abnormalities in the brain’s neurotransmitters, serotonin and norepinephrine—chemicals key to mood and widely recognized for their role in depression. Not all patients with fibromyalgia, however, have depression or respond to antidepressants. Treatment studies of the other types of antidepressant drugs, including selective serotonin uptake inhibitors and tricyclic agents, have had mixed results.

A new and different antidepressant, duloxetine, works by inhibiting the reuptake of both serotonin and norepinephrine. In a recent clinical trial conducted for the treatment of fibromyalgia—one of the largest ever—duloxetine was shown to reduce pain and improve a range of disease symptoms, significantly and safely. The results, published in the September 2004 issue of Arthritis & Rheumatism, offer the promise of relief for women with fibromyalgia.

“Our results suggest that duloxetine improves pain and tenderness, the hallmark characteristics of fibromyalgia,” states Lesley M. Arnold, M.D., who coordinated the research at 18 centers, including the University of Cincinnati College of Medicine, Indiana University Medical School, and Harvard Medical School. “The effect of duloxetine on the reduction of pain,” Dr. Arnold further notes, “appears to be independent of its effect on mood.”

To test duloxetine’s effectiveness on the range of symptoms, researchers recruited 207 patients, all meeting the American College of Rheumatology criteria for fibromyalgia. Like the majority of those with this disease, the majority of the participants—89 percent—were women. 87 percent of the subjects were Caucasian and the mean age was 49. Just over a third of the patients—38 percent—had been diagnosed with depression. On a random basis, the patients were prescribed one of two treatments for a course of 12 weeks. About half, 104 individuals, received 60 milligrams of duloxetine twice a day. The remaining 103 patients were given a placebo. Both groups were evaluated and scored, using the Fibromyalgia Impact Questionnaire and other standard measures, for improvements in their condition.

In various measures of disease—from pervasive pain to tiredness to tenderness—the female fibromyalgia patients treated with duloxetine improved significantly over those treated with a placebo. One of the most dramatic changes was in the reduction of the number of tender points—places on the body where it hurts to touch—and the increase of pressure pain threshold. Women with or without depression receiving duloxetine benefited emotionally and physically, reporting improvements in general mood, ability to function, and overall enjoyment of life.

For the study’s 23 men, however, duloxetine did little to change their condition. Although researchers reported some evidence of improvement in tender point measures among duloxetine-treated men over their placebo-treated counterparts, it was not statistically significant. “The reasons for the sex differences in response are unclear,” Dr. Arnold observes. “Because the male subgroup was small, reflecting the much higher prevalence of fibromyalgia in women, the results of the study may not be generalizable to all men with fibromyalgia. There may also be sex differences in fibromyalgia that affect treatment response.”

As Dr. Arnold notes, further research is needed on larger samples of not only men but also other groups with fibromyalgia to evaluate duloxetine’s effectiveness.

Duloxetine (Cymbalta®) is indicated by the FDA for the treatment of major depressive disorder and is not indicated for the treatment of fibromyalgia.

Fibromyalgia Syndrome (FMS)

For the most part, routine laboratory testing reveals nothing about fibromyalgia. However, upon physical examination, the fibromyalgia patient will be sensitive to pressure in certain areas of the body called tender points.

To meet the fibromyalgia criteria for diagnosis, patients must have: Tender point diagram used for the fibromyalgia diagnosis, basedon the CDC's 1994 guidelines.

A. Widespread pain in all four quadrants of their body for a minimum of three months

B. At least 11 of the 18 specified tender points (see diagram*)

The 18 sites used for the fibromyalgia diagnosis cluster around the neck, shoulder, chest, hip, knee, and elbow regions. Over 75 other tender points have been found to exist, but are not used for diagnostic purposes.

While many chronic pain syndromes display symptoms that overlap with fibromyalgia, the 1990 American College of Rheumatology (ACR) multi-center criteria study (published in the February 1990 issue of Arthritis and Rheumatism) evaluated a total of 558 patients, of which 265 were classified as controls. These control individuals weren't your typical healthy "normals." They were age and sex matched patients with neck pain syndrome, low back pain, local tendonitis, trauma-related pain syndromes, rheumatoid arthritis, lupus, osteoarthritis of the knee or hand, and other painful disorders. These patients all had some symptoms that mimic fibromyalgia, but the trained examiners were not foiled—they hand-picked the fibromyalgia patients out of the "chronically ill" melting pot with an accuracy of 88%. Fibromyalgia is not a wastebasket diagnosis!

Although the above diagnosis focuses on tender point count, a consensus of 35 fibromyalgia experts published a report in 1996 saying that a person does not need to have the required 11 tender points to be diagnosed and treated for fibromyalgia (Wolfe F, et al. J Rheumatology 23(3):534-9, 1996). This criteria was created for research purposes and many people may still have fibromyalgia with less than 11 of the required tender points as long as they have widespread pain and many of the commonly associated symptoms below.

  • fatigue
  • irritable bowel (e.g., diarrhea, constipation, etc.)
  • sleep disorder (or sleep that is unrefreshing)
  • chronic headaches (tension-type or migraines)
  • jaw pain (including TMJ dysfunction)
  • cognitive or memory impairment
  • post-exertional malaise and muscle pain
  • morning stiffness (waking up stiff and achy)
  • menstrual cramping
  • numbness and tingling sensations
  • dizziness or lightheadedness
  • skin and chemical sensitivities
