Annual injection lowers osteoporosis fracture risk

NEW YORK (Reuters Health) - A once-a-year infusion of zoledronate, which goes by the brand names Zometa and Reclast, can significantly reduce the risk of fractures of the spine and hip in postmenopausal women with brittle bones due to osteoporosis, according to a report in this week's New England Journal of Medicine.

In earlier studies, yearly treatment with zoledronate was shown to improve bone density, but it was unclear if this translated into a significant reduction in fracture risk.

In the current study, Dr. Dennis M. Black, from the University of California, San Francisco, and colleagues assessed the occurrence of fractures in 3889 women, average age 73 years, with postmenopausal osteoporosis who were randomly assigned to receive annual treatment with zoledronate or an inactive placebo.

Compared with placebo, zoledronate reduced the risks of vertebral, hip, and other fractures by 70 percent, 41 percent, and 25 percent, respectively.

In general, zoledronate therapy was not associated with any more adverse effects than the placebo. However, while uncommon, serious atrial fibrillation -- in which the upper chambers of the heart beat rapidly but weakly -- affected significantly more patients in the zoledronate group (50) compared with the placebo group (20).

In a related editorial, Dr. Juliet Compston, from the University of Cambridge School of Clinical Medicine in the UK, comments that while the findings are encouraging, "the practicality and acceptability of annual intravenous therapy in large numbers of women remain to be tested."

Nevertheless, she writes, "Increased treatment choices for patients are to be welcomed and may provide one means of improving adherence and treatment outcomes in osteoporosis."
