Plastic Surgery Procedure Restores Virility to Prostate Cancer Patients

November 5, 2001 – Men who have their prostate gland* removed (prostatectomy) due to cancer can still maintain their sexual function through microsurgical nerve grafting, according to a study presented this week at the 70th Annual Scientific Meeting of the American Society of Plastic Surgeons (ASPS) in Orlando.
According to the study, up to 80 percent of cancer-related prostatectomy patients sustain some type of injury to the cavernous nerves. Cavernous nerves, found on both sides of the prostate gland, become engorged with blood during sexual arousal and are responsible for erection.

"Many men lose their ability to have an erection after a prostatectomy because critical cavernous nerves are partially or completely severed during the procedure," said Rahul Nath, M.D., lead author of the study and assistant professor of plastic surgery at Baylor College of Medicine in Houston, Texas. "Using nerves from a patient's leg and attaching it to the severed cavernous nerve creates the potential for the nerve to regenerate."

According to Dr. Nath men with damage to both cavernous nerves have zero potency, while men with one severed nerve have 17 percent potency. "Potency numbers are significantly increased in patients that undergo nerve grafting, from zero to 55 percent potency in patients with both nerves severed and from 17 to 71 percent in patients with one," he said.
The study examined 24 patients with both cavernous nerves severed and 51 patients with one severed nerve. All underwent microsurgical nerve grafting to restore erectile function. The procedure was developed by Dr. Nath and colleagues in urology at Baylor College of Medicine. A control group of 54 patients who did not undergo nerve grafting but had either one or both cavernous nerves severed was used.

After one year, 49 of the men who underwent nerve grafting (13 patients with both nerves severed and 36 with one nerve severed) experienced spontaneous erections sufficient for unassisted intercourse.

The study concluded that patients with severed cavernous nerves may regain full erectile function if microsurgical nerve grafting is performed immediately following the prostatectomy.

"To achieve erectile function in about 65 percent of our study group is quite an achievement," added Dr. Nath. "Most men who experience cavernous nerve damage after prostatectomy never fully regain their virility."

"Prostatectomy surgery can lead to a loss of sexual function, which could be devastating for a patient," said Saleh Shenaq, M.D., co-author of the study and professor of plastic surgery and neurosurgery and chief of the division of plastic surgery at Baylor College of Medicine in Houston, Texas. "It is our belief that this procedure will give the majority of patients hope to a return to life as they knew it before being diagnosed with cancer."

The American Cancer Society estimates more than 198,100 men will be diagnosed with prostate cancer in 2001.

 
Many Accidents Call for Replantation Surgery

Over the summer, a shark attack on an 8-year-old boy required replantation surgery to reattach his arm. There are unfortunately many accidents where a replantation is appropriate to consider. This type of surgery is not new. The first successful arm reattachment (replantation) surgery was performed on a 12-year-old boy in Boston in 1962, and the first thumb reattachments in the United States and Japan in 1967.

During the summer months, lawn mower, power saw and other injuries also frequently occur to the hands and fingers. Any time a finger, arm or hand that has been completely cut off from the body is surgically reattached, it is referred to as replantation. Replantation can be attempted only when the amputated part has not been extensively damaged. The alternative to a replantation may be another type of local reconstruction, a prosthesis or a substitute device.
